Michigan Smoke-Free Air Law Complaint Form Process


The Michigan Smoke-Free Air Law went into effect on May 1, 2010.  This historic public health law protects Michigan residents and visitors from secondhand smoke exposure in restaurants, bars, and businesses, including hotels and motels. 

 

If you observe a possible violation, please notify the owner or manager of the establishment when feasible, and give them the opportunity to take the appropriate steps to resolve the situation before a formal complaint is filed. If the violation is not resolved, you can report the violation for further action.

·         Submit completed form to the Chippewa County Health Department by mail, fax, or hand deliver to the CCHD Environmental Health office.    

·         Complaint forms may be submitted anonymously however, if you would like a reply back from the CCHD, please include your name and contact information.

·         Unless required by law, all complaints received by our department are confidential
