(2) FULL-TIME REGISTERED CASE MANAGER: Under the supervision of the Home Health Nursing Supervisor, performs duties using professional judgment. Provides skilled nursing care under the order of a physician in a client’s place of residence and acts as a case manager for the client/family and develops the plan of care with coordination of all disciplines as determined by the nursing assessment. Ensures quality nursing services are rendered to all clients in accordance with the standards of the Health Department. The employee in this position must be able to maintain effective working relationships with supervisors, co-workers, clients, and general public in all areas of the Health Department. The employee must be able to prioritize, multitask, and work independently. Must be able to follow written and oral instructions. The person in this classification must be able to maintain confidentiality. On-call after orientation is required, travel throughout Chippewa County and a reliable, insured vehicle are also required. Staff receive mileage reimbursement at the IRS-approved rate. Hourly rate: $22.28 – 27.10.
REGISTERED NURSES: Full-time registered nurse and a part-time registered nurse are needed for our Personal & Family Health division. These positions include community-based and clinic-based program duties. Must be licensed as a registered nurse in the State of Michigan, and have a driver’s license. Full-time position provides a complete benefit package.
PART-TIME Disease Intervention Tech/Case Investigator: Primary purpose is to support department work for contact tracing and case investigation to support mitigation and prevention of continued spread of COVID-19 as a part of pandemic preparedness and response strategies. Responsible for assisting with existing or suspected cases of COVID-19 and possible contacts, majority of which will happen via telephonic communication. College experience preferred. Days and hours needed are: Monday – Friday between 8:00 a.m. – 4:30 p.m. $17.00 per hour
