Job Summary

Under the supervision of the Home Health Nursing Supervisor, performs duties using professional judgment. Provides skilled nursing care under the order of a physician in a client’s place of residence and acts as a case manager for the client/family and develops the plan of care with coordination of all disciplines as determined by the nursing assessment. Ensures quality nursing services are rendered to all clients in accordance with the standards of the Health Department. The employee in this position must be able to maintain effective working relationships with supervisors, co-workers, clients, and general public in all areas of the Health Department. The employee must be able to prioritize, multitask, and work independently. Must be able to follow written and oral instructions. The person in this classification must be able to maintain confidentiality.

Essential Job Functions

• Thorough knowledge of nursing theory and skills
• Ability to plan and schedule skilled nursing
• Ability to educate and train staff and/or clients and their families
• Excellent documentation skills
• Good communication skills
• Must be able to lift, bend, stoop, push, pull and transfer clients, with good body mechanics

Job Requirements

• Associate diploma or bachelor’s degree in nursing
• Current licensure to practice nursing in the State of Michigan
• Prefer minimum of two years experience in medical-surgical nursing
