Plan Review Instructions Fixed Food Establishment
Section 8-201.11 of the 2005 FDA Food Code requires that plans and specifications be submitted for review and approval before construction of a food establishment, conversion of an existing structure for use of a food establishment, or remodeling of a food establishment if the Health Department determines that plans and specifications are necessary to ensure compliance with the code.
The plans and specifications must include the following:

Completed application.

Plan review fee of $649.00 payable to the Chippewa County Health Department.

Completed Plan Review Worksheet. Use "N/A" where an item is not applicable.

Menu.

Standard Operating Procedures (SOP"s). Standard Operating Procedures are required for all establishments. See Page 3 of Plan Submission Instructions.

One complete set of plans.

Equipment list and specifications.
Construction may not begin until the requested material has been submitted and the plans have written approval.
