• Plan Review Instructions Special Transitory Food Unit

    Section 8-201.11 of the 2005 FDA Food Code requires that plans and specifications be submitted for review and approval before construction of a food establishment, conversion of an existing structure for use of a food establishment, or remodeling of a food establishment if the Health Department determines that plans and specifications are necessary to ensure compliance with the code.


    The plans and specifications must include the following:

    1. Completed application.

    2. Plan review fee of $314.00 payable to the Chippewa County Health Department.

    3. Completed Plan Review Worksheet.  Use "N/A" where an item is not applicable.

    4. Menu.

    5. Standard Operating Procedures (SOP"s).  Standard Operating Procedures are required for all establishments.  See Page 3 of Plan Submission Instructions.

    6. One complete set of plans.

    7. Equipment list and specifications.

    Construction may not begin until the requested material has been submitted and the plans have written approval.
