Job Summary
EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION/LICENSURE:
Education: Licensed Bachelor’s Level Social Worker (LBSW)
Experience: Minimum one year experience in health care or similar field preferred.
Other Requirements: Current license to practice in the State of Michigan. Maintain a current CPR certification. Position requires travel around Chippewa County; reliable transportation necessary. Possession of a valid Michigan driver's license and vehicular insurance.

ESSENTIAL FUNCTIONS:
1. Assesses the psychosocial status of the client and communicates findings to the interdisciplinary team.
2. Carries out social determinants of health screenings, depression/anxiety screenings and other applicable assessments and directs interventions based upon evaluation findings.
3. Maintains clinical records on all clients
4. Provides information and referrals for services applicable to client needs.
5. Provides information to clients or families/caregivers regarding community agencies.
6. Serves as liaison between clients or families/caregivers and community agencies.
7. Maintains collaborative relationships with personnel from CCHD and other private and public agencies (ex. Great Start Collaborative and Early On, Child Protective Services) to support client care.
8. Participates in the development of the total plan of care and case conferences as required.
9. Participates in community-based advisory councils/coalitions with a focus on family support.
10. Participates in discharge planning.
11. All other duties as assigned.

Adheres to Health Department telephone courtesy standards for answering, placing and transferring calls, as well as general telephone use. Does not allow personal telephone calls or visitors to interfere with telephone responsibilities, public encounters or customer courtesy to others. Demonstrates an effort to assure that customers/clients wait in definite expectations of when and how they will be served. Professionally addresses others (smile, eye contact, voice, posture, grammar, grooming, and dress). Responsible for promoting and securing potential clients and referrals.
